关于事故上报，我们邀请您以回答问卷的形式来进行。以下为简要说明：

本问卷，一共分为七个部分28道题。分别为一、基本情况（1-3题），二、关于人员（4-6题），三、关于设备（7-10题），四、关于活动（11-16题），五、事故等级（17-23题），六、概要描述（24题），七、个人信息（25-28题）。

其中，
第五部分，17-23题里，只需要选择一个答案即可。
第七部分，除了手机号做验证用必填，其他题目为非必答，但我们建议您能填写，由您来决定。
