本表用于了解机构或个人的会员加入意向、专业背景及合作需求。专委会目前面向全球征集首批会员加入意向，不收取会员会费。

提交本表后，秘书处将根据您填写的信息，与您沟通会员申请、参与方式及相关合作安排。

填写本表仅代表提交加入意向，不等同于正式入会、专家入库或获得具体项目参与资格。


Thank you for your interest in the Green Sustainable Hospital Committee of the World Green Design Organisation (WGDO-GSHC).

This form is intended to help us understand the membership interests, professional backgrounds, and potential areas of collaboration of institutions and individuals. The Committee is currently inviting expressions of interest from its first cohort of members worldwide. No membership fee is charged.

After you submit this form, the Secretariat will review the information provided and contact you to discuss the membership application process, ways to participate, and potential collaboration opportunities. 

Please note that submitting this form constitutes an expression of interest only. It does not constitute formal admission as a member, inclusion in the Committee’s expert database, or eligibility to participate in any specific project.
