INFORMED CONSENT FORM (ICF) TEMPLATE
(VERSION No. 03-2023)
1. KEY INFORMATION ABOUT THE RESEARCHERS
Title of the Study: Dormitory Services and Management Practices and Their Influence on Learning Motivation and Effectiveness: Bases for an Enhanced Dormitory Development Pla
Name of Researcher/s: Wang,Tiantian
Research Adviser: Dr. Bibiana Espina
Department/College: Graduate Studies
Institution: Central Philippine University
2. INTRODUCTION/BACKGROUND OF THE STUDY
You are invited to take part in this research study. This form contains information that will help you in deciding whether to participate or not in this study/research. Before you decide to participate in this study, you will be given enough time to read and understand the contents of the informed consent. If there are words or concepts that you do not understand feel free to ask questions at any time, the researchers are willing to explain them to you and your questions will be answered to your satisfaction. The study will begin once you have signed the informed consent form.
Against the backdrop of China’s “Sanquan Education” strategy and deepening higher education reform, university dormitories have evolved from mere living spaces to critical extensions of classroom learning and social interaction, especially with the widespread adoption of student-centered pedagogies like Problem-Based Learning (PBL). While basic dormitory amenities (e.g., private bathrooms, Wi-Fi) are increasingly standardized, gaps persist in academic support, personalized services, and collaborative management—gaps that hinder students’ learning motivation and effectiveness. This study seeks to address these gaps by exploring how specific dormitory services and management practices shape undergraduates’ learning outcomes, with findings to inform an evidence-based enhanced dormitory development plan that aligns residential environments with holistic talent cultivation goals.
3. PURPOSE OF THE RESEARCH
The purpose of this research study is to investigate the current status of dormitory services (encompassing basic living security, safety guarantee, learning support, and social and cultural development) and management practices (including institutional, guiding, and collaborative management) in a Chinese public university, examine their influences on undergraduate students’ learning motivation (intrinsic interest, goal motivation, group drive) and learning effectiveness (course pass rate, knowledge application ability, learning time utilization rate), and ultimately develop an enhanced dormitory development plan based on empirical findings to optimize the dormitory’s role in holistic talent cultivation.
4. TYPE OF RESEARCH INTERVENTION/DATA GATHERING INSTRUMENT
Choose researcher-made and adapted questionnaire.
For Quantitative Research:
- This quantitative study adopts a researcher-made questionnaire as the core data collection instrument, which is independently developed by the researcher based on an extensive review of existing relevant literature and tailored to the actual context of dormitory management and services in Chinese public universities. The questionnaire consists of a total of 65 items scored on a 5-point Likert scale, including 6 items of demographic information and 59 items measuring core research variables, and is divided into five parts in total. The specific contents of each part are as follows: the first part is demographic information, collecting basic personal and living background data of participants; the second part is dormitory services evaluation, measuring participants’ perceived quality of dormitory services from four dimensions of basic living security, safety guarantee, learning support and social and cultural development; the third part is management practices assessment, evaluating the implementation effectiveness of dormitory management from three dimensions of institutional management, guiding management and collaborative management; the fourth part is learning motivation measurement, gauging participants’ learning motivation level from three dimensions of intrinsic interest, goal motivation and group drive; the fifth part is learning effectiveness gauging, assessing participants’ learning effectiveness from three dimensions of course pass rate, knowledge application ability and learning time utilization rate.
5. PARTICIPANT SELECTION (INCLUSION & EXCLUSION CRITERIA)
You are chosen as a participant based on the following criteria:
Age range: 18–22 years old (consistent with the typical undergraduate age in Chinese public universities for this study);
Gender: Male or female;
Academic status & living experience: Officially registered full-time undergraduate student at a Chinese public university, with at least one academic year of campus dormitory living experience;
Cognitive & behavioral ability: Able to complete the questionnaire independently without communication or cognitive barriers, and willing to voluntarily participate in the study.
Exclusion: Individuals who fail to meet any of the above inclusion criteria will be excluded from the study.
6. VOLUNTARY PARTICIPATION /RIGHT TO REFUSE OR WITHDRAW
Your participation in this study is entirely voluntary. It is your choice whether to participate or not. If you choose not to participate or to withdraw from the study at any time, there will be no penalty or other consequences, and no need to give any reason. Your data will be discarded properly.
7. PROCEDURE
As a participant, you will be required to independently complete the self-designed quantitative research questionnaire in a quiet on-campus environment. You need to answer all the items in the questionnaire truthfully and according to your actual situation in strict accordance with the filling instructions attached to the questionnaire.
8. DURATION OF THE STUDY
The overall implementation period of the study is from January 2026 to June 2026, which is consistent with the research scope, limitations and the time schedule in the Gantt chart. The time required for each participant to independently complete the questionnaire is approximately 25 minutes.
9. RISKS AND INCONVENIENCES
This study will pose an insignificant or low risk, as the questions are not sensitive.
10. BENEFITS
This study delivers tangible benefits across stakeholders: participants gain self-reflection on their academic well-being, while universities obtain data to refine support services like stress counseling and tutoring. Educators can adjust teaching approaches to foster a more supportive classroom, and future researchers gain a validated framework for exploring mental health and academic performance. These outcomes create a cycle of improvement that enhances the student experience and strengthens institutional effectiveness.
11. REIMBURSEMENTS
To acknowledge the time and effort of the respondents, a small token of appreciation will be provided upon the completion of their involvement in the study. The specific form of the token of appreciation is stationery (a ballpoint pen).
12. CONFIDENTIALITY
The information you have provided is solely for this study. Your identity will be kept private and confidential to the extent provided by law. You will be assigned an ID number and your data will be stored with utmost respect to your privacy.
13. DECLARATION OF CONFLICT OF INTEREST
The researcher declares no apparent conflict of interest in the conduct of the study.
14. STORAGE AND DISPOSAL OF RESEARCH DATA/MATERIALS
All collected research data and materials (including paper questionnaires, signed informed consent forms, electronic questionnaire data and data analysis files) will be stored in a strictly confidential manner, and only the core members of the research team have access to the above materials and data. Paper materials will be kept in a locked special filing cabinet, and electronic data will be stored in a password-protected and encrypted dedicated computer with double backup. In accordance with the institutional data management policies of Central Philippine University, all research data and materials will be retained for five years after the completion of the study (until June 2031). After the 5-year retention period, all paper materials will be completely shredded with a paper shredder, and all electronic data will be permanently deleted from all storage devices and backup media, with no residual data retained.
15. SHARING OF RESULTS/DISSEMINATION PLAN
The research findings will be disseminated through multiple channels: the researcher will notify all participants of a public forum for sharing the study’s key findings via on-campus official announcements; hardbound copies of the research report will be available in the university library for student respondents to access, and the published version of the research will be released online for public viewing. In addition, the findings will be shared more broadly through academic publications and national/international academic conferences.
