     您好!本研究旨在了解非音乐专业大学生在陶笛公选课中的学习自我效能与审美体验。本问卷为前测问卷,将在10周课程正式开始前填写,用于了解您在课程学习前的基本情况、对器乐学习的预期感受及相关背景信息。您的参与对本研究具有重要意义。
在填写本问卷之前,请您知悉以下事项:
  1. 自愿参加:本问卷的填写完全出于自愿,您有权决定是否参与本研究。
  2. 匿名保密:问卷采用匿名方式填写,您所提供的所有信息仅用于学术研究目的,不会以任何方式泄露您的个人身份。
  3. 自由退出:在填写过程中,您可以随时终止作答,无需说明理由,且不会因此受到任何不利影响。
  4. 编号机制:完成本问卷后,您将获得一个专属序号标记。请妥善记录该序号,在课程结束后填写后测问卷时需使用同一序号,以便研究者将前后测数据进行配对分析。该序号不包含任何个人身份信息。

Pre-test Questionnaire — Informed Consent

Dear Students,
      This study aims to investigate the learning self-efficacy and aesthetic experience of non-music major university students enrolled in the ocarina public elective course. This is the pre-test questionnaire, to be completed before the 10-week course begins. It is designed to collect your baseline information, including your background, expectations regarding instrumental learning, and related personal experiences prior to the course. Your participation is highly valued.
Before completing this questionnaire, please be informed of the following:
  1. Voluntary Participation: Your participation in this study is entirely voluntary. You have the right to decide whether or not to take part.
  2. Anonymity and Confidentiality: This questionnaire is anonymous. All information you provide will be used solely for academic research purposes and your personal identity will not be disclosed in any form.
  3. Right to Withdraw: You may discontinue the questionnaire at any time without providing a reason, and this will not result in any adverse consequences.
  4. Identification Number: Upon completion of this questionnaire, you will be assigned a unique identification number. Please record this number carefully, as you will be required to use the same number when completing the post-test questionnaire after the course, so that the researcher can match the pre-test and post-test data for analysis. This number does not contain any personal identifying information.
  5. Course Participation Requirement: Participants are required to complete all 10 weeks of the course and to fill in the post-test questionnaire. During the course period, please refrain from taking leave unless absolutely necessary, so as to ensure the integrity of the learning experience and the validity of the research data.
